Everyone makes mistakes but owning up to them is a different matter. It is quite hard to admit the errors and harder to apologize for them. This is a common thing in medical history where hospitals always discouraged doctors and nurses from admitting their mistakes. This fear is due to the fear of precipitating a malpractice lawsuit and also due to embarrassment and the discomfort due to the process. In full disclosure, the doctor explains why the error happened, how its effect will be minimized and the steps that will be taken to avoid recurrence. Medical error is a human error which causes damage to the patient. The role of a healthcare organization is crucial in error disclosure. Patients normally want an explicit statement that why the error happened and what measures are being taken to minimize the impact, they don't expect an apology but a complete plan on the aftermath of the error, so it is the responsibility of the organization to take the patient in their confidence and do the needful. The doctors and nurses prefer to stay silent, and they normally prefer not to disclose the information, this is done because of the pressure from the organization. Mistakes are part of healthcare practices and they cannot be avoided all the time, the biggest fear of physicians is that once they tell the patients then it might happen that more people will know about that and ultimately no patients will come to visit them, and if l know about the error then they have to face legal issue, and the situation will become worse. These are some of the most observed gaps certain measures must be taken.
