Shedding Light on Chronic Disease via Community Health Workers
Introduction and Background
In America, CHWs aid in meeting the set national objectives by carrying out activities on a community level. Interventions have also held that belief in progressing health and preventing disability and diseases. CHWs are basically known by various names, some names inclusive are health promoters, peer leaders, peer health advisors, community health representators and so on. With regards to the expression by the CHWs section of American Public Health Association, CHWs are basically front health workers of the public who are entrusted members and have a unique understanding of the community that they serve. This relationship based on trust aids CHWs to work as an intermediary in-between social or health services and the society to accommodate accessibility to various services and the improvement of the quality and the cultural capability of the service deliverance. CHWs are also responsible for building individual and community dimensions by growing health awareness and making people self-sufficient via a variety of activities, for instance, advocacy, social support, outreach, society education, and informal counseling (Yanek et al., 2016). Additionally, CHWs also provide healthcare education to healthcare givers. They help them with the awareness of various cultures and how to use communication skills. CHWs also aid in reducing the costs of healthcare by the linkage of patients with community resources and avoiding unnecessary hospitalization.

The graph shows the reduction of diseases in regards to a five-year mark. It is evident that CHWs prevention control has been aiding in the reduction of various diseases. There was a 10 reduction seen in the deaths due to chronic disease it is also said that by this year an additional 12 reduction will be seen. The cardiovascular disease went through one of the biggest reductions (Brownson et al., 2017). According to the US statistics of cancer for the current fifteen years, it was seen that a great number of people died because of the illness, but there was a reduction seen in by time. it has a lot to do with how the disease is managed now. Further, the illness does not have the same impact on all ethnicities. It was seen that African American s were more likely to pass away because of the illness than whites. When it came to respiratory diseases like asthma, it continues to be one of the major concerns of the society. The disease cannot be cured completely, but due to recent development, the management of it can be done very well, which is seen in the chart. There have been many fatalities in the past due to the disease, but things are gradually but consistently becoming better. The highest rates of asthma occur in children, multi-race, females, Black Americans, Indian Americans and the natives of Alaska. Around 9.3 of the American population has been diagnosed or are yet to be diagnosed by cancer. Without proper management, people can develop type two diabetes. However, the management is being done very efficiently, and lesser people are passing away because of the illness. Further, it needs to be kept in mind that a disease can have a different impact on an individual based on their ethnicity. A white might have a different problem as opposed to a Hispanic. Further, it is seen that there is evident improvement seen in the charts seen when it comes to the diseases that are being faced nowadays and have been present for quite some time. that can be due to the advancement of technology as well, but CHWs is providing impactful support, prevention and control of all of these diseases.
Measures to Control and Prevent the Diseases
It is clear that CWHs can aid in overcoming hurdles to the controlling of chronic diseases. Below are the measures that can be taken
The need for cultural awareness is essential in order to mediate communication between the healthcare giver and the patient.
Proper and popular education methods are used to educate the healthcare givers in regards to cultural diversity. Culturally appropriate and accessible healthcare is to be provided to the inpatient.
To ensure the fact that people get the service that they are in need of.
Giving of informal counseling and social support to the patients.
Supporting societies and individuals.
Trying to structure and build community and individual capacity.
Providing the individuals of the society with outreach.
Measuring and keeping under observation the blood pressure.
Educating the families about their rights.
Helping the individuals navigate the healthcare system they will be using.
Giving social support by listening to the concerns of the individuals.
Constructing community clinical links.
The burden of chronic diseases can be explained through the diseases mentioned above. When it comes to the pharmaceuticals, treatments, research, and funding. the research has been increasing day by day for chronic diseases such as cancer, diabetes, cardiovascular disease and respiratory disease like asthma. While there is proper treatment and funding present, but it faces biasness when it comes to other ethnicities (Dieleman et al., 2016). As time is changing the resources are getting minimal as well. The healthcare in return is becoming very expensive, within the next five years the resources will mostly be exported and become very rare. Healthcare, especially in the US, will become more biased. It already is towards blacks, Hispanics and Asians but it will increase by the time. the treatments are progressing especially cancer as targeted therapy is now present, but it is very expensive. Additionally, insulin has become very progressive as well. The injection that a diabetic person had to use twice a day now can go with a bigger gap without it. Better healthcare setups, sugar check instruments, surgical instruments, less biased facilities, and more aware healthcare givers are required. Since the Affordable Care Act was signed by Obama, there was progression seen. 20 more people got health insurance, but thinks are on stagnant at the moment, and issues are being faced. The resources are being provided by the act, but the ethnicities are still not inclusive. At least, a 20 more increase is required in the resources being provided.
