I. Health Status
a. Introduce the health issue (disease or problem)
Substance abuse is a problem that can be defined as a harmful pattern of substance intake.it includes the abnormal intake of alcohol and another drug. It can result in abuse because the magnitude that is being taken is not recommended and it will lead to critical health issues, and social issues. The most commonly used illegal drugs in the United States are Cocaine, Heroin, Marijuana, Molly, and hallucinogens. (Horgan, Skwara, Strickler, Andersen, 2004).
East Orange Substance Abuse program provides treatment to a patient by full filling their clinical needs. This health issue is common in very common in the state and the usage of opioid was found both in males and females. The organization provides support to the patient with the help of its helpful staffs (East Orange, NJ, 2019).
b. How does the health issue affect the target population
Substance abuse affects the target population of the United States in various ways. It makes them socially isolated, economically dependent and is extremely harmful to their health. It could be fatal and could cause lifetime disability. It creates mood disorder, thinking disorder, personality disorder and effect the mental health of an individual. (Horgan et al., 2004).
II. Community Description
a. Describe the setting
The organization provides state of the art, specialized and scientific based treatment to the patients and their families. The organization provides a clinical environment that is congruent with the needs of the patient who are having mental issues and physical issues due to substance abuse (East Orange, NJ, 2019).
b. Describe the target population (i.e. age, ethnicity, disease, race, gender, sex, etc.)
The target population is all the adults in New Jersey in particular and the adults of the United States in general who is going through substance abuse. According to the statics of New Jersey drug addiction, 67 percent of the substance abuse patients are male and 32 percent are female. Now 70 percent of the teens are involved in drugs either they are buying or selling it (NJ Drug Rehab, 2019).
III. Needs Assessment
The substance abuse rate in New Jersey is very high. The state ranks in 39th position in term of substance abuse and the alarming thing about this issue are that a very high number of school students up to 30.7 percent are involved in substance abuse (NJ Drug Rehab, 2019). Thus my agency service will be helpful to overcome the increasing rate of substance abuse.

This report presents the increasing intake of heroin and also discuss the fatal and non-fatal effect of the drug on an individual (STLT Gateway, 2019).

16,651 death was recorded in 2010 due to the overuse of opioid and thousands of government intervention was applied to control this. Finally, the country is controlling the abuse of opioid (Dart et al., 2015)

The agency is in partnership with The East Orange Water Commission in order to manage municipalities (East Orange, NJ, 2019).
c. What barriers (do you believe) currently exist that limit your organizations ability to reach more of its intended population
The agency could not be able to spread throughout the state which is not letting it reach to its intended population.
