What is a patient-centered medical home? What are the criteria that must be met to achieve certification as a PCMH? How does the electronic health record play a central role in the PCMH?
In the PCMH, the care is coordinated both with the components of the health care system (specialties, hospitals, etc.) and with the patient's environment (family, local public or private services, etc.); the care activity relies on information systems to make sure that patients take the proper care; access to care is facilitated by appropriate schedules and the development of contacts by email or telephone. PMCH is a medical practice. The concept of PCMH include actively supporting patients to take care of themselves (patient's self-management), calling on non-physicians to manage care, having a device for reminders to doctors for preventive acts, to systematically follow the results of analysis and to identify abnormal results, to follow the addresses to specialists or to the hospital, to use electronic prescription tools with safety controls to measure the performance of physicians.
To gain PCMH Certification, it is required to fulfill the training and continuing education requirements. The 30 CEU points are required for this purpose. It can be renewed through NCQA. It’s also advisable to keep a record of the Continuing education. The exam can be retaken to renew the certification. Without this certification, the existence of PCMH is impossible.
Without timely and secure access to complete and reliable information, the PCMH can be ineffective, depriving the patient of the best possible care. By systematically grouping electronic health data, clinics, EHRs connect hospitals, community hospitals, provincial and regional health establishments and other specialists in the field and provide them with an overview of patients' medical histories, no matter where they are.
